Dear Parents and Students,
We want to let you know about one of the greatest
events of the summer! This summer our
students entering 7th and 8th grades (in the fall 2015)
will have an opportunity to kick off the summer by attending one week at Forest
Home Camp and Conference Center in Forest Falls, CA. We will be attending Creekside—an excellent
camp designed just for Jr. High students.

When
is all this fun happening?
The dates of this year’s camp are Sunday, June 14
- Friday, June 19. The cost is $460. A non-refundable deposit of $200 is due by
May 10th. The balance will be
due May 20th.
My
student has special medical needs. What
do I do?
Once registered online, you’ll be supplied with
an email that will have a link to a document called “Important First Aid
Message to Youth Pastors & Parents.” Please read this letter to see how the
camp can help you with any medical needs you may have. You may need to fill out a Special Medical
Needs form (pages 5-6 of this document). Please note that we do NOT need pgs 5-6 from every family; it is only applicable to
particular medical situations.
Scholarship
Information: If the price is prohibitive, please consider that
we have scholarships available, just send me an email and I’ll get you the
scholarship request form. We will not accept scholarship forms after May 4th.

Camp spots are very limited, so sign up
quickly…to reserve your spot just provide us with the following:

Jr. High Summer Camp Registration Questionnaire (registration available online)
- Forest
Home Registration and Medical Consent Form (link provided to this Form in confirmation email: complete pages
3-4)
- If you are applying for a scholarship,
send me an email for the scholarship request form.
- If your student has special medical needs,
submit the Special Medical Needs Procedure Authorization Form (link provided to this Form in
confirmation email: complete pages 5-6).
Payment and paperwork will be received online and
via scanned documents emailed to BBathauer@hillsiderancho.com, at
the Jr. High section of our information booth after the weekend gatherings, or
at the church office until May 10th (that will be the last day to
sign up!). Once you register, we will provide you with a packing list,
transportation information, the check-in procedure, and the camp’s contact
information. Final payment will be
accepted online, at the information center, and the front office through May 20th.
